New Patient Registration
We are pleased to accept new patients living within our practice area. We cover an area with boundaries including parts of Reigate and Dorking in the north, extending to some areas of Capel, Rusper, Faygate, and Charlwood in the south. We therefore include North and South Holmwood, Beare Green, Newdigate, Leigh and Norwood Hill. You can check to see if you are in our boundary by entering your postcode into the tab entitled Practice Boundary. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill.
We normally register patients at the surgery they live closest to. We do not restrict patients' choice to one GP, this means patients can choose to see any of our doctors and may even choose to see different doctors for different problems. However, all patients are allocated a named GP who is responsible for your overall care.
We will require sight of photographic ID such as a passport or photo driving licence. For a new baby/child we will need to see the form FP58 issued by the registrar or a birth/adoption certificate. New entrants to the UK are required to provide proof of settlement by way of a visa or other documentation.
To register you will need to complete a registration form and a health questionnaire (for patients aged 16 and over) which will provide us with useful information whilst we wait for your medical records to arrive from your previous surgery.

New Patient Health Check
We offer every new patient that registers with us a new patient health check. This is a 20 minute appointment with the Practice Nurse who will check your height, weight, BMI, blood pressure, urinalysis and discuss your family history.
